How to fill out a Work Order

Work Order template preview
Free Work Order template

A work order is a document that authorizes and records a specific job, such as a repair, installation or maintenance task. It describes the work requested, who is assigned, the labor and parts used and the final charges, and the customer signs to confirm the work was completed.

How to fill it out

  1. Enter the customer's name, contact information and the service address.
  2. Fill in the work order number, date, assigned technician, scheduled date and priority.
  3. Describe the problem or work requested, including equipment model and serial number.
  4. List labor (hours × rate) and parts and materials (quantity × unit price) — line amounts and the total are calculated for you.
  5. Enter the sales tax rate if it applies, record the completion date, and have the technician and customer sign.

Tips

  • Write down the equipment model and serial number — it makes warranty claims and repeat visits faster.
  • Get the customer's approval before doing work beyond the original request, and note it on the work order.
  • Sales tax on labor and parts varies by state; in some states repair labor is taxable and in others it is not.
  • Number work orders in sequence so you can match them to invoices.

FAQ

What is the difference between a work order and an invoice?

A work order describes and tracks the job itself — what needs to be done and what was used. An invoice requests payment. Many businesses turn a completed work order into an invoice.

Who fills out a work order?

Usually the business or maintenance department creates it when a request comes in, and the technician completes the labor, parts and completion details.

Can a work order be used for internal maintenance?

Yes. Facilities and property managers use work orders to track repairs inside their own buildings; just leave the pricing columns blank if no one is billed.
