A credit memo (credit note) is a document a seller issues to reduce what a customer owes, for example after a return, damaged goods or a pricing error. It references the original invoice and shows the items and amount credited.

How to fill it out

  1. Enter your business details and the customer’s name and address.
  2. Fill in the credit memo number, original invoice number and date, and account number.
  3. Check the reason for the credit.
  4. List the items credited with quantity and unit price — amounts and total calculate automatically.
  5. Choose how the credit will be applied and sign.

Tips

  • Number credit memos in their own sequence (e.g. CM-1001).
  • Always reference the original invoice so the customer’s records match.
  • Use the same tax rate that was charged on the invoice.
  • Keep a copy with the original invoice in your records.
