A change order is a written amendment to a construction or service contract. It describes added, removed or changed work, prices it, shows how the contract sum and completion date change, and is signed by both parties before the work proceeds.

How to fill it out

  1. Fill in the project, address, change order number, owner, contractor and contract date.
  2. Describe exactly what work is changing and why.
  3. Price each item with quantity and unit price; enter credits as negative quantities.
  4. Enter the original contract sum and prior change orders — the new contract sum calculates automatically.
  5. Record any schedule change and have both parties sign.

Tips

  • Sign the change order before starting the extra work.
  • Reference drawings, specs or locations so there is no doubt about scope.
  • Number change orders in sequence and keep a log.
  • Some states have special rules for home improvement contracts — check yours.

FAQ

Who signs a change order?

Usually the owner (or client) and the contractor, and sometimes the architect or engineer.

How do I show a credit for deleted work?

Enter a negative quantity for the deleted item; the total reduces automatically.

Can I export it to Excel?

Yes. The Excel file keeps the amount and contract-sum formulas.
