Why are permission slips needed?
Most schools and districts require written parent consent before students leave campus. The slip also tells families what the trip involves and gives staff the emergency information they need while away.
Can the slip authorize emergency treatment?
Yes. The signature section includes consent for staff to get emergency medical treatment if a parent can't be reached. For longer or overnight trips, some schools also use a separate medical consent form.
What if a parent doesn't return the slip?
Most schools don't let a student go without a signed slip; a phoned-in or emailed permission is accepted only if your district allows it. Plan supervision at school for students who stay behind.
Can I use it for clubs, scouts or church groups?
Yes. Change the organization name, leader and group, and edit the transportation and description to match your outing.
