A medical consent form for a minor lets parents authorize another adult — a grandparent, relative, coach or caregiver — to approve medical, dental or emergency treatment for their child during set dates. It records the child’s medical information and insurance.

How to fill it out

  1. Enter the child's full legal name, date of birth and home address, and each parent or guardian's name and phone number.
  2. Name the authorized caregiver (grandparent, relative, sitter, coach or trip leader) with their relationship, phone and address.
  3. Set the Valid From and Valid Until dates and a short reason, such as "summer stay with grandparents" or "school band trip."
  4. Fill in allergies, current medications with dose and timing, medical conditions, immunization status, the child's doctor and the insurance policy and group numbers.
  5. Sign and date it. If it will be used for travel or by a school or camp that asks for notarization, sign in front of a notary.
  6. Give the original to the caregiver along with a copy of the front and back of the insurance card, and keep a copy yourself.

Tips

  • List medications by name, dose and time ("albuterol inhaler, 2 puffs as needed") so the caregiver can tell a doctor exactly what the child takes.
  • Write allergies as specifically as you can, including the reaction (for example, "peanuts — hives and swelling, carries EpiPen").
  • Use a fixed end date that matches the trip or stay. Make a new form for the next one instead of leaving it open-ended.
  • If the child takes daily medicine, also send a short dosing schedule — the form records what they take, not when to give it.
  • Both parents can sign when both are available; many providers accept one parent's signature, but two avoids questions.

FAQ

Does a medical consent form need to be notarized?

Usually not, but notarization makes the form easier to accept, and it is often requested for travel, especially when the child is traveling without either parent. A notary block is included at the bottom.

Who should have one?

Anyone caring for your child when you can't be reached quickly: grandparents or relatives during a visit, a regular babysitter or nanny, a coach or trip chaperone, or a family friend during your own travel.

Will a hospital treat my child without it?

U.S. hospitals will treat a life-threatening emergency without parental consent. For non-emergency care — stitches, an urgent care visit, a dental problem — providers usually need consent from a parent or someone the parent authorized in writing.

Is this a travel consent letter?

No. This form covers medical treatment. If a child travels abroad with only one parent or with another adult, airlines and border officers may also ask for a separate travel consent letter.

Is this legal advice?

No. This is a general template, not legal advice. Laws differ by state and city, so check your local rules or ask an attorney before relying on it for anything important.
