How to fill out a Job Application Form
Free Job Application Form template
A job application form is a standard document employers use to collect the same information from every applicant — contact details, the position, availability, education, work history and references. This one-page form avoids questions that many states prohibit on applications.
How to fill it out
Enter your full name, phone, email and address.
Fill in the position, start date, desired pay, availability and employment type.
Answer the work authorization and visa sponsorship questions.
List your education, skills and licenses.
Add up to three recent jobs with dates, duties and reason for leaving.
List references, read the certification, then sign and date.
Tips
Employers: do not ask about age or date of birth, race, religion, national origin, marital status, disability or other protected traits.
Many states and cities have “ban-the-box” laws that limit when employers may ask about criminal history, so this form leaves that question out.
Several states and cities also ban asking about salary history, so the form asks for desired pay instead.
Applicants: use the same dates and job titles as on your résumé.

FAQ
Why is there no question about criminal history?
Many states and cities have “ban-the-box” laws that delay or restrict criminal history questions until later in hiring. Leaving it off the application keeps the form usable in more places.
Can employers ask if I am authorized to work in the U.S.?
Yes. Employers may ask whether you are legally authorized to work in the U.S. and whether you will need visa sponsorship. Proof of eligibility is collected on Form I-9 after hiring.
Can I attach a résumé instead?
Many employers accept a résumé along with the application. Fill in the form anyway so all applicants are compared the same way.
