How to fill out a Performance Review Form
Free Performance Review Form template
A performance review form documents how an employee performed over a review period. This template rates seven core competencies with weights, calculates an overall score on a 1–5 scale and records strengths, areas to improve, goals and employee comments.
How to fill it out
Fill in the employee, job title, department, reviewer, review period and type.
Give each competency a weight (all weights add up to 1.00) and a rating from 1 to 5.
The weighted scores and overall score calculate automatically.
Write specific strengths, areas to improve and goals with target dates.
Discuss the review with the employee, add their comments and sign.
Tips
Base ratings on examples from the whole period, not just recent events.
Use the same competencies and scale for everyone in the same role.
Make goals specific and measurable.
Export to Excel to keep the weighted score formulas.

FAQ
How is the overall score calculated?
Each rating is multiplied by its weight; the weighted scores are added up. With weights totaling 1.00 the result is on the same 1–5 scale.
Does the employee have to agree to sign?
Signing usually confirms the review was discussed, not agreement. The employee can add comments.
Can I change the competencies?
Yes. Every competency name is editable and you can add rows.
