An exit interview form collects honest feedback from an employee who is leaving. It records the main reason for leaving, rates key parts of the job and asks open questions so the employer can spot problems and improve retention.

How to fill it out

  1. Fill in the employee’s name, job title, department, manager, dates and interviewer.
  2. Check the main reason for leaving.
  3. Rate each area from Poor to Excellent and add comments.
  4. Answer the open questions in your own words.
  5. Sign if you wish; the form is kept by HR.

Tips

  • Have someone other than the direct manager conduct the interview so answers are more candid.
  • Make participation voluntary and explain how answers are used.
  • Look for patterns across several exit interviews, not single answers.
  • Hold the interview in the last week, not on the final hour.
