How to fill out a Medication Log

Keep all your medication information on one page. The top section lists allergies, your doctor, pharmacy and emergency contact; the Current Medications table records each medicine’s strength, schedule, purpose, prescriber and start date; and the Daily Log tracks each dose taken with time, initials and notes. It is a record-keeping tool, not medical advice.

How to fill it out

  1. Fill in the name, date of birth, allergies, doctor, pharmacy and an emergency contact.
  2. List every current medication, including vitamins and over-the-counter medicines, with strength, how often and why.
  3. Each time a dose is taken, log the date, time, medication and dose and check Taken; caregivers can add initials.
  4. Note side effects or missed doses, and bring the log to appointments and pharmacy visits.

Tips

  • Copy names and doses exactly from the pharmacy label.
  • Update the list whenever a medication is started, stopped or changed.
  • Keep a copy in your wallet or bag for emergencies and new doctors.
  • This log is not medical advice — always follow your doctor’s and pharmacist’s instructions.
